Botulinum toxin

Botulinum toxin is a chemical protein produced by the Clostridium botulinum virus. It irreversibly attaches to the presynaptic nerve terminal to inhibit the secretion of acetylcholine at the nerve junction, thereby preventing muscle contraction and achieving the effect of muscle relaxation.

Application

Botulinum toxin is the most known naturally occurring biological agent in humans. Quantitative injections of botulinum toxin are used. For example, for treating neuromuscular disorders characterized by hypersensitive skeletal muscles by providing muscle paralysis. Symptoms treated with botulinum toxin type A include facial spasm, adult-onset spasmodic torticollis, anal fissures, blepharospasm, cerebral palsy, dystonia of the neck, migraines, strabismus, temporomandibular joint disorders, and various muscle spasms. Recently, it has been used for wrinkle treatment, facial muscle spasms, muscle contraction therapy, and facial plastic surgery.

How It Works

The "toxin" part of its name comes from its ability to paralyze muscles. It does this with incredible precision by targeting the neuromuscular junction-the point where a nerve communicates with a muscle fiber.

Here is a step-by-step breakdown of its mechanism:

Blocks Signal Release: Normally, a nerve ending releases a neurotransmitter called Acetylcholine (ACh). ACh crosses the gap to the muscle fiber, binding to receptors and telling the muscle to contract.

Targets SNARE Proteins: Botulinum toxin works by cleaving (cutting) specific SNARE proteins, which are essential for the vesicle containing ACh to fuse with the nerve cell membrane and release its contents.

Prevents Contraction: By destroying these proteins, the toxin prevents the release of ACh. Without this chemical signal, the muscle cannot contract, leading to flaccid paralysis (limp, relaxed paralysis).

This effect is not permanent, as the nerve eventually sprouts new endings to re-establish the connection, a process that typically takes 3 to 6 months.
